I was uninstalling some apps to get ready to back up and flash my phone because my microphone volume was stuck at minimum and no one could hear me. After rebooting my back and multitask (menu) button won't light up or respond. This has happened to me before occasionally. I could fix it by powering the screen off and back on. I believe I read somewhere it had something to do with xposed.
I tried flashing using MOP and my buttons didn't come back so I loaded a TWRP, wiped all partitions, rebooted in download mode and flashed with regular ODIN. Still can't get the buttons to function. Home button works just fine, though.
I tried finding a solution to this but I can't seem to find one. I had the same issue intermittently on my Galaxy S4 but I fixed it by wiping the partitions and reflashing. G900R4 USC variant.
So I went into stock recovery and did a factory reset. It was already as wiped as it could possibly get afaik but I thought I'd try just to see. My buttons lit up in stock recovery so I knew they were getting power. I did a factory reset and low and behold upon reboot my buttons came back.
Did I fix it or is there a circuit starting to fail on my phone and it was just luck? I'm curious.

A message popped up from Samsung to install an update (I had been pressing cancel for the past week on this update). It says click OK to reboot and apply update (something along those lines) so I do. Now I am stuck with a tablet that won't boot. If I hold the power button it says "Samsung Galaxy 10.1" after about 12 seconds, but then it goes away and back to darkness. Nothing else. Holding power and volume up does nothing.
Someone know what is going on and have any idea how to resolve?
The best way is to restore any backup you have from cwm recovery. Else grab a stock firmware and flash it with ODIN or if zip than CWM recovery. For luck, try clearing cache and data in recovery, first!Perhaps it may work saving you all fuss of flashing a firmware.
I cannot even boot that far too get into recovery. And I just noticed that although I can't boot, the backlight is on (on a solid black screen).... wtf

Nope. I plugged into my computer and for some reason with the USB connectd it let me enter recovery. Flashed a backup and rebooted, but now I have no wifi... im confused.
it occured once to me when i tried to overclock, it never worked except when installing a new rom... Try to root the device and then install a new rom
OK, was a helluva night messing with ROM and kernel installs... As a side note, when I say the wifi is not working, it still shows all networks and allows me to connect just like normal. However when connected no apps can access the network...
Can someone point me to a thread on how to revert EVERYTHING back to stock so I can start fresh? TIA!!

Should have all the files you need. From where you are now I would flash the stock OTA 3.1 or TW then flash the stock recovery and do a factory reset from that recovery. That should get rid of the garbled screen.

I heard the update was pulled as it sometimes rewrites the bootloader improperly. You might need to send it in, unfortunately.
So basically my phone was stock but rooted and modded slightly with volume boost, camera mod, and the multi window mod as well. Other than that its all stock and never flashed a Rom on it. The phone ran smooth about 90% of the time but there were little quirks here and there that kind of bugged me. For example, every once in a while the screen wouldnt wake up after being locked via button or double tap. Wifi wouldnt respond randomly unless I reset my phone. Things of that nature, but anyhow I decided maybe hard reseting my phone might clear up some of these issues. I got into TWRP from AutoRec, and checked off everything except internal storage. I believe they were Davlik, Cache, System and obviously it didnt work out for me. I feel like me checking off the system is what did it because it wasnt considered a basic swipe at that point. Anyhow I cant get to recovery, download mode or anything of that sort. Everytime I hold down the power button it just resets and brings me back to the LG logo and just sits there while the led lights keeps rotating colors. Is my phone done for?
edit: Oh yea one last thing, I remember when I went to reboot it I somehow ended up being asked if I wanted to install SuperSU and I said yes. And than at that point it went to restart my phone and got stuck.

You wiped system. That's your rom. Don't wipe system unless you plan on flashing a rom. You should still be able to get back to recovery. From phone powered off state ( hold power and volume up when lg logo is on). Hold power and volume down, when lg comes on, release both, then hold both again. You should now be at the factory reset screen. Press power three times (it's not going to factory reset). You should be in recovery. From here, restore your backup. If you don't have a backup, you just found out the reason why you should always have a backup. If you don't have a backup, you're going to need to install a rom via adb or otg (assuming your recovery is up to date). You can also tot or kdz back to stock (wipes sdcard though).

so my sister has an att galaxy s4. And she started having this problem with it only booting into download or factory mode. So after two days of struggling I finally got it to boot normally only after doing the download mode button combo, and selecting for it to cancel the boot. Then it kept on doing it after every boot so I thought i would flash the stock firmware from odin. This did not work as well and I kept getting "fail". Now I did an over the air update and as far as I can tell, it is still having the issue. Also I cannot get into recovery with the button combo to delete the cache. Thanks for the support and any help would be greatly appreciated as I do not know where to go with this.

Update: after the update it has now started to boot successfully without the extra steps, will report here again if it reoccurs.
problem still persisting
Well now the phone can boot into the normal stock Rom just fine, only after the previous method is done. Phone does not stay off after powering down and just reboots automatically into download mode. Thanks for any help guys, no idea what to do now.
Kind of sounds like some of the buttons are sticking. Maybe the power button (why it won't stay off) and/or the volume button rocker switch. You could try repeatedly pressing to clear whatever is making it stick. Also you can open the phone up and use a can of air to clean out the buttons. The only thing I can think of that may be causing this issue

i currently have a galaxy S5 SM-G900F which is stuck in safe mode,i have searched and searched this forum for a solution and have exhausted all possible fixes for this issue
the only one i have not yet done is to use odin to flash a new firmware...yet
this device was dropped into a toilet and after drying it out was scrambled,(the rom kept jumping into all sorts of different parts of itself,so i updated from kitkat via kies to lollipop...the update went fine and the device has re-booted without the previously explained problem above but now it will not exit out of SAFE MODE
if anybody has any suggestions as to a fix that would be very appreciated....also the menu and back hardware buttons light up but do not work only the home button seems to work and am not sure if this is an effect of safe mode or not...also VOL- seems not to work either
could all of these problems be related to that fatal toilet dunk and will it be worth my while getting these parts fixed or replaced...???
The button problem should appear because of the toilet drop.

this much info i did gather from XDA...now the device is back to going all funky again...will only boot if vol+/home button/power button are held together then as soon as it boots the system goes haywire...flickering between all sorts of crap and so fast that i cannot identify anything...only a battery pulls seems to solve that issue...is weird as it was working fine this afternoon straight after i updated to lollipop..for hours it was fine...then funky rom raised it's ugly head and has not gone away...also it is still in safe mode as well....
If you have done this already then boot your phone into Recovery Mode, wipe cache and do a factory reset, I can't remember but I think in stock recovery (I use TWRP so I can't check) but I think there is an option to shut down the phone so if it stays in recovery after the wiping and factory reset select that option, then remove the battery for a few minutes, then replace the battery and reboot the phone.
Hopefully the water hasn't caused any permanent damage, they are only waterproof to a limit and it can get in, the non working buttons could be related to a conflict with 4.4 rom residue software and the new 5.0 software, the wipes will clear this, then if its still not behaving try to re-flash the rom again, if that fails then the water may well have got to it.
Bear in mind the factory reset will wipe all phone data, contact data, etc, basically it will restore it to an out of the box condition.

So yesterday i was just cassualy using my phone, when suddenly my volume down button started to act weird.
It kept pushing itself and raise or lower the volume. Since i didnt update my phone in a while, i figured it might be a software issue.
So i installed the latest update, and suddenly my phone booted in safe mode. Tried to reboot, no luck. Now my volume down key is unresponsive... I opened the phone up and cleaned the ports, but still no luck...
Looks like the key is dead. Now i am rooted and have twrp installed.
What are my options here?
Can i install an android 9 rom, maybe they wont boot into safe mode, i'm not sure?
I do still have warranty, so i could try my luck with relocking the phone and sending it back, but i also already opened it to look at the volume keys.
Do you guys have any ideas on how to handle this?
