How do I get my 16 GB memory card back...it now only shows 200 MB - HD2 Windows Phone 7 Q&A, Help & Troubleshooting an

I put WP 7 on my HD2 and it was working fine but I decided to go back to Android. I then took my 16 GB Sandisk card out and put it in the memory card reader but it only shows the partition of 200 MB.
I even download a program to format/partition the microSD card but I still could not get it to get back most of the available space in my 16 GB card. It keeps showing 200 MB on my computer and I tried to get rid of the partitioning but I still cannot do it.
Is there a solution to this?

sounds like your memory card is corrupted :/

Try
http://www.sdcard.org/consumers/formatter/eula/
If that doesn't fix it, it may be borked completely.. though some people have had success with the Nokia phone formatting tools

Easy,
Put your SD Card in your PHone
POwer off you phone
During Boot hold power off/on button on phone to access the MAGDLR Menu
Connect you Phone to PC
Choose MASSSTORAGE in MAGDRL Option number 4
Format your SD CARD over PC
Power off Phone - Job done!

i had the same problem, the way the i fix it was the i format the sd card in my blackberry and it works for me i try it in a samsung and fix the problem. the computer wasnt able to fix problem i used the format method from the computer. didnt work, and i used paragon software didnt work either. the only way was with different phone

I don't know why everybody thinks you have to put the card into something else. Enter MAGLDR, go to USB, connect it to the computer, run SD Formatter,Select correct drive, Options - Format Type= Full Erase On -Format Size Adjustment =ON.
This has been covered many times since the introduction of WP7. Using the search term 200mb in this section. I came up with plenty of answers to your problem.

THE SDHC EncFiltLog file CORRUPTION SOLVED

PROBLEM:
Now, the problem all started when try to format the micro SDHC on the phone or on the PC with card reader.
Tried with differ format program also low level not worked.
DONT USE THESE!!! Waste of time!!!
SOLUTION:
So after doing some thinking about the situation, I figured that there was a problem with the card inserted and find the file EncFiltLog on it.
It is the WM 6.1 encription file. You can't delete, move files from or in to the SDHC.
The best way to solve this issue, as the hard-reset DOES NOTHING to fix this issue, would be to have windows hopefully find the connected device and format it. Here are the EASY steps which DO NOT involve using a dozen computers and has provided the same result EVERY time; which is a working on my 8gig memory in my HTC HD.
1. Connect the HD to the computer and select DISK DRIVE as the option on the on-screen popup on your phone. Don’t select ACTIVE SYNC. If you miss it, then go into your phone and press START>SETTINGS>CONNECTIONS TAB>USB to PC and select the bullet of DISK DRIVE and hit ok.
2. On your computer, (USING A WINDOWS XP MACHINE), go into your "My computer"
3. A window for will open with two columns.(If not, change the view to "Group") We are looking at the RIGHT side right now. In there is has three categories, and we are only focusing on ONE of them. LOOK for STORAGE which should be the center category.
4. Click with right to the desired "Storage device; A: ; D: E;", it will take a minute or two depending on your computer speed and environment to populate your hard drive and connected memory devices.
5. You will eventually see your PHONE internal storage listed. IT WILL TAKE SOME TIME BECAUSE YOUR PHONE INTERNAL STORAGE IS CORRUPTED!
***BE SURE WHAT YOU ARE LOOKING AT IS IN FACT YOUR PHONE AND NOTHING SOMETHING ELSE LIKE YOUR MAIN HARD DRIVE OR SOME OTHER STORAGE DEVICE WITH IMPORTANT FILES ON THERE***
****I TAKE NO RESPONSIBILITY IF YOU FORMAT THE WRONG DEVICE***
6. Choose format and let it format the drive. This should also take a few minutes. Select FAT32 and name is whatever you want! Let this finish COMPLETLY!
**Your almost done**
7. Now you can copy, anything you want on it, but if you put again in to the PDA then the
EncFiltLog will come on again. But at least you not have to throw away the card,
you can use it again.
8. Finally my last comment:
If you want to speed up the card (.cab installation, copy, move ...etc.....)
the best way after all this work listed above, you should made a BACKUP from all your PERSONALITIES and
make a HARD RESET.
After it should speed up all the programs and processions with the card.
If you done then one IMPORTANT THING!:
YOU SHOULD NEVER EVER TAP ON THE CARD ENCRIPTION IN THE SETTINGS,
NOT EVEN THINK ABOUT IT!!!!!!
Hope this helps you all out!!
BTW: I want to say thanks to XDA-FORUM (all people) to make my life easier
and to found the right gadgets to use, to play and enjoy it....
Wow - not bad at all for a first post!
Welcome to the forum!
Thanks for providing a solution!

Yesterday it happens for the second time that my SD-Card gets corrupt again, the first time it happens with a 8 Gb SDHC Sandisk and now with a 16 Gb.! After a hard-reset.Didn't know before I read this post that this could happen ever!??
And indeed I couldn't write or read on the card or format it again, even with a cardreader!
Both cards are useless about €120,- thrown away, gone
Didn't know before I read this post, that this is an WM 6.1 encription issue. Never used or even tap it.
Now I format another 8Gb like the way you describe here, i hope that when i need to do another hard-reset i can use the card again!
BTW, its a little bit strange to me that there are so little reaction from other users here about EncFiltLog problem?
I hope that there will come a solution in WM 6.1 to prevent that b!llsh!t.
Thanks for the great support, i hope it realy works!
Nice post
I have one more question:
What may happen when I delete this encryption file? Is it possible?
P.S. Sorry for my english :/
Hi Everyone!
The best way, if you purchase a new card, to FORMAT at first.
Then you put in to your phone and try not to teke it out any more.
If you want to use it as USB reader the as I write in my first post
START>SETTINGS>CONNECTIONS TAB>USB to PC and select the bullet of DISK DRIVE and hit ok .
In this case you can use the phone as a card reader.
It is speed up the file transfer vs. ACTIVESYNC.
If the card is not recognised ether way then,
because of the too much taking out - - put it back; went to faulty.
You can go back to the store for guarantee replacement.
The EncFiltLog is a problem only if you encrypted the card.
The solution to delete it and use as new card described in my first post.
If you put a new card in to the Phone then this file seem to be created by the OP system.
You can delete with format, but it will come back again and again.

Your right, the file is still there again!
But I have also a question about a next( hope not) hardreset on my device!
Is it better to format the SD-card (with USB on the device) before or after the hard-reset to save the further use of the card?
Because in my experience you have to start installing everything over again, because the data on the card was anyway corrupt and useless.
It is NOT recommended to use the MS Windows Format command to re-format any SDHC card - it will format them, of course, but with the wrong cluster size, slowing access speed down and possibly causing other problems.
I would suggest to always use a tool like Panasonic's SD Formatter when formating any kind of SD card. You can get it here: http://panasonic.jp/support/global/cs/sd/download/sd_formatter.html

Is there no registry tweak to turn off or delete that W 6.1 encryption option anyway?

No as far as I know.
Will creating a read-only, hidden folder called "EncFltLog.menc" on the card prevent this problem?

What do you mean? How do you do?
HI folks, glad to find some peeps who are suffering from the same problem...
The solution here seems not to work for me, as I hard-reset my TouchHD before I found out about that encryption ****up...
Is there any solution that allows you to reformat the card?
Not to mention that I NEVER touched that damn encryption option as I really never trust M$ that far with my data... seems it got me Pwned anyways...
Anyone ever tried to Write M$, tell them what a bunch of wacky Ho's they are and demand a new miniSD Card from them?
sorry for my language but atm I mad as hell as I just lost a 8GB and a 16GB sanDisk...
I experienced a lot of bc during all the yaers messing with M$ stuff but this ****up ins unbelievable...
I had the same problem with my 8gb sandisk.
I would like to add that I was able to retrieve most of my files from the memory card using EASEUS Data Recovery Wizard Professional. You need to do the data recovery before attempting to reformat the card.
Hi guys sorry for messing around in your forum but this post is the closest solution to my problem. I cannot format or save anything to my 16gb sandisk card because of that bloody encryption file. Now the problem is that I'm a Kaiser user and I do not have disk mode in my phone, so has anyone successfully formatted their card without using disk mode?
one very quik question. Does this fix the problem for good? you can use the sdhc card with the phone or will it always end up with menc file?
card unreadable in one night
Yesterday, my sdhc 8gb card was working properly but in just one night something happened to it. In the morning, I came to know that it is not more being recognized by htc hd2. Every time I soft reset or hard reset the phone a message come up saying "Do you want to format this storage card to make it readable? this will permanently delete any files on the card." when I click no nothing happens and phone keeps on working and if I click yes then the card is formatted in just 1 second. I tried formatting the card myself from phone format tool (there is format card tool in tools section of the phone) but it is showing only 30.61 mb instead of 8gb.
After reading this thread, I think the problem is with EncFiltLog. Actually I backup my phone and card yesterday with sprite backup. I also backup EncFiltLog on my pc. Now after hard resetting the phone I delete the newly created EncFiltLog file by the fresh WM 6.5 installation after hard reset. I deleted that file from my pc by using the phone card storage as disk drive instead of activesync and simply shift+delete the file from removable storage in My computer. After that I copied all the phone card backup (including EncFiltLog - old one) on the freshly hard reset phone. Now all of sudden in just one night phone sdhc card is not able to recognized by phone.
Moral of the story: Never backup EncFiltLog. Keep the newly created EncFiltLog file by wm 6.5 after hard reset and copy your all files and folder from card backup to phone card (excluding EncFiltLog) after a hard reset. Don't ever overwrite the newly created EncFiltLog.menc file with old one from backup.
Reason: I think WM 6.5 pairs the newly created EncFiltLog with its some key on phone internal storage. Remember how encryption works? I am talking about public key and private key pairing. I think WM6.5 pairs one key stored in EncFiltLog with another key stored inside the phone interal storage which is itself freshly created after the hard reset but when we copy the old EncFiltLog from storage card backup to freshly hard reset phone storage card, those key pairs won't match anymore and windows mobile 6.5 simply locks up the card.
it is my own opinion.
One more clue
Ok guys, I've got one more clue after googling and reading other threads on this forum. May be HTC is supplying fake 8gb sdhc card with hd2.
I've came on that conclusion because yesterday there was more than 5 gb space left on my phone storage card. In the night RSS Hub software has updated all the feeds and stored those feeds on storage card (I've set feeds to be stored on storage card). I think something has been gone wrong there. May be the actual capacity of this fake storage cards in far less than what is written/claimed on the card (i.e. 8gb). In the night as soon as the RSS Hub stored feed on the card, it somehow corrupted the card FAT writting its data on sectors of the card where it should not be.
Hence the card is corrupted now just showing 30.61 mb instead of 8gb. There exists no solution for this situtation till now. I've sent mail to HTC regarding this on their customer support page. Let's see what would happen now. I suspect either HTC or my dealer would say that the storage card don't come under warranty. Let's see now. I will update the thread soon.
My problem is the same.....
after HSPL I faced "Do you want to format this storage card to make it readable?..." and after that I lost my mSD.
Format Storage Card didn't worked. it shows 30.61MB , Cluster Size: 0KB
also Clear Storage didn't worked...
System > Memory > Storage Card > Total storage card memory: Not installed
Is there any other way to fix this problem? because of my country, it is difficult to use HTC Service...

16 hours and I have only one issue left.. memory card is only being recogised 200mb

I have been at this for 16 odd hours and have overcome wifi probs, activation probs, chevvy issues.. done 3 flashes of wp7, radio updated.. all is grand now but the memory card issue!
Please, someone allow me to go to bed by explaining how I can format the card to show more than 200mb?!
I have taken the card out, formatted on my pc, reformatted with other tools, am stuck, and have spent the last hour searching through threads for info I had earlier but can no longer find! it doesnt help that any search containing "SD" is ignored as xda search sees it as too small a word.
Who wants some good Karma?
I really dont get why this topic is been creating/cloned etc on a regular basis??
Please use search function, this topic has been discussed many times.
Use some commen sense, dont look for "SD", use keywords like "WP7" and "200" or "200mb", "partition" etc. Or even reading the readme that came with the WP7 rom would be a good start
Why not help me instead of witholding what you know and making me forrage around one hell of a mess of a forum to find information I have been searching for since this afternoon????????
from the read me, and i quote "WARNING: WP7 will format the SD card during the first boot.
Two SD card partitions are created during cold boot.
First one is a 200Mb FAT partition and another one with unknown format which
is used as RAID with main memory."
That doesnt explain how to change the RAID partition into physical memory I can access from the device...

Here is what you need to do:
Make sure your SD card is in the phone
Do a factory reset by going to settings>About>Reset your phone.
After WP7 reboots, it will recognize your card and all the date.
You will have to re-enter your Activation code.

Thanks for the advice.
Is there any way this can be done outside of the device? I have heard that I can get an SD formatting tool but whatever one I downloaded did nothing to help.
I am reluctant to do the 4th hard reset of the day for this OS as a) there is talk of the auth code working only 3 times, and b) I have everything else working now and dont want to tempt fate!
Is it only a WP7 hard reset that will resolve this for me now?
:-(
Thanks in advance of your help my man!!
also, wont a hard reset undo all of the unlocking and such from chevvy?

Im pretty sure it will undo the unlocking, but it only takes a few minutes to go thru it again once you have everything downloaded.
Hard reset number 5 it is then!!
It didnt save my settings on the mem card, obviously as it wasnt in there in the first place during the previous hard reset.
Im becoming a master at hard resets and personalisation now!
that's not gonna help, , the first time it boots its gonna again make a 200meg partition and the rest create a raid.
download a partition program (there are loads, personally i have a ubuntu laptop and use that, you could also make an ubuntu live cd, boot to that and delete teh partition there.
through Ubuntu...
phone into magldr, usb mass storage, connect to Ubuntu, the 200meg filesystem icon will soon appear on the desktop, right click, format, instead of clicking ok click disk utility, select the 200 partition and click unmount then click delete, then select the unknown partition, and click delete.
if you are going back to android now would be a great time to set up an ext partition if you want one.
If you are sticking with wp7, then you can't format it to show more than 200meg, that's all that wp7 allocates to the fat partition, all the rest goes to system storage. Messing with the disk (even removing it whilst wp7 is running, i believe) will trash the data on the card and wp7 will reformat it.
If you are heading back to wm then create a fat32 partition the full size of the card.

[Q] MicroSD card Mount fail after format

First off let me say Thanks to all the developers and testers for their hard work. It gets a bit frustrating when you have few words to say but are unable to do so because you don't have 10 posts.
I started messing with my HD2 after noticing what a bs windows mobile 6.5 truly was. I was fortunate enough to find the scene the week nand rom flashing became the hot new way to "free" our devices.
Few weeks have passed since, and slowly below the radar, community has moved towards CWM roms. New sick way of customizing to your heart's content. Thanks to everyone involved in development of this sick tool.
Now back to original reason for this post. I am running CWM 1.3 on my Leo 1024 HTC HD2. The last rom I tested was Revolution HD v2.5 CWM. prior to this I have tested about 15 different roms. 100% success rate and no issues.
I am also running class 10 MicroSD 16 GB card from Kingston. After flashing so many times the card got a bit fragmented so I decided to do a format using Panasonic SD formatter v2.0. After the format was done, my card fails to read anything I put on it, from the CWM (when I try to install zip rom it gives me "failed to mount /sdcard (Invalid argument)" and "can't mount /sdcard".
At the same time If I mount USB storage from CWM when connected to my laptop, it comes up as external storage and I have full acess to cards contents.
What the hell did I do?
Sounds to me that you need to format it with an actual program like FDISK under Windows. Often times, programs that can format things for you have problems. Google for instructions on how to format your SD card via the Windows Command Prompt using the FDISK utility.
Thank you ashasaur, I'll give it a try!

Well, It seems problem was ID10T after all. Usually I am really good at paying attention to the details but this time it slipped by me...
It seems there is absolutely no issue with the Panasonic SD Formatter after all. The issue that followed after the attempted format of the MicroSD card was due to the wrong format setup.
I am posting proper instruction on how to format and avoid the NOOB situation I got myself into. This will also work for people that flashed WP7 and decided to go back to Nand, but tought SD card is ruined due to the WP7 protection mechanism.
Note: This will only work if your SD Card is atleast visible to your PC.
KEEP PHONE PLUGGED INTO YOUR PC THROUGHOUT THIS WHOLE PROCESS.
1. Reboot Phone and HOLD VOLUME DOWN KEY to enter BootLoader Screen (multicolor screen)
2. Download and flash MAGLDR 1.13. Reboot Phone
3. When phone is rebooting HOLD POWER BUTTON to enter MAGLDR Screen.
4. Go to OPTION 4. USB MassStg in MAGLDR.
5. Now Download SD Formatter 3.0 and install it.
*If your PC is not recognizing your SD Card go back to MAGLDR Main Menu by pressing the Power Button once and go to OPTION 10. RESET with phone still plugged in, and hold Power Button down while rebooting to get back to MAGLDR screen and select option 4. USB MassStg again. It should recognize it.
6. Open SD Formatter 3 and make sure your SD Card is being shown in the Drive Dropdown Box.
7. You can enter whatever Volume Label you like. Then click the OPTION BOX.
8. Select Format Type - Full Erase, and select Format Size Adjustment - On.
9. Then click Format and your SD Card should be reformatted and ready for use again.
It worked for me and I hope it works for others
*I didn't write these directions and as such I don't take credit for it. Directions were zipped with SDFormater3.0 (user didn't sign it). Thanks, however you are!

[Q] Help with partitioning SD Card

Hello first I want to say a big THANK YOU to everyone that has put time and effort into making the Nookie what it is today....but I have a noob question.
I recently got a 16gb micro SD and can successfully get the 0.6.8 Nookie to run on my nook color, my problems start when I try to "recover" my other 13gb's. Before I get burned and tossed aside I have checked
http://forum.xda-developers.com/showthread.php?t=922324&page=92
http://forum.xda-developers.com/showthread.php?t=883175&highlight=partition&page=103
and about 5 other pages and they all say the same thing..use EASEUS, grab the slide bar and voila. It appears to work but when I install it in the NC it either becomes very unstable or just boots to the NC as if the SD card was not there.
Anyone have any ideas?

I use Lexar Bootit ....a free utility...to format and flip the bit that says its a removeable drive, then your windows pc drive management can see and format it as full capacity fat32.
Hey thanks for the tip, unfortunately it won't work. I forgot to mention that I followed this tutorial ..... and updated my driver (saved my original Windows just in case) to a hitachi driver and now I can switch my SD memory card forth "removable" to "logical" and back when needed. Once I get it as a logical drive I was EASEUS to increase the size of partition 4 (SD Card)..but again no dice!!!
Seeing that I am a noob I can't post the link to the tutorial but I'll say it worked like a charm and when I want to go back to the original I just need to "roll back" the driver.
Well, a couple of things:
Are you using a Sandisk card? Sandisk class 2 and 4 cards have been found much more stable than most others for running a ROM from SD, and problems with SD installs often go back to the card.
Any particular reason you went with Nookie Froyo? It has pretty much fallen out of use since the NC's stock OS was updated to Froyo in May. If you want an SD install, there's a simpler, size-agnostic method for CM7, which is also a more robust and capable OS and a more advanced version of Android (Gingerbread 2.3.4 rather than Froyo 2.2). I would recommend CM7.1 RC1, or if you're adventurous, Nightly 136.

Thanks for the info unfortunately I am using a 16gb PNY, class 4 and at $15 I couldn't pass it up...looks like I should have done some more reading first. Really I have no clue why I am using the Nookie, my wife said said that was the one she wanted. I think she just likes saying "Nookie", but I'll give the CM7 a go and see what trouble I can get into.
thanks again...
Taosaur...I was able to get the CM7 mod up and running in no time flat. Now the wife wants me to see if I can get bluetooth working.
You said "I was able to get the CM7 mod up and running ...." so I assume you got your problem fixed. That's good.
About bluetooth.
1. Turn off Wifi
2. Power off NC
3. Power on NC
4. Turn on bluetooth
5. Turn on wifi

Yep, I had to do the above the first time I turned on bluetooth, but haven't had any trouble toggling it on and off since then. Just open notifications (that broken-circle-and-arrow button on the status bar) and you have toggles there for Wifi, bluetooth, and a couple other things.
@ Taosaur,
I've seen your signature indicate you are running nb136, any change or improvement (both slightly and/or significant) over the previous ones?

I haven't done much but read and browse a little since I flashed it, but I haven't noticed any difference from 7.1 RC1, which wasn't really any different from Nightly 102 I had before that. I mostly just flashed this time for the integrated OC/Tweaks kernel, without the video/lag problems that were reported in 132-134.
Thanks for an update, m8
maybe someone can help me, i'm about to loose my mind here trying to figure this out. i'm using a Sandisk class 4 16gb card running CM7 with the OC kernal.Did all of this using the size agnostic install method. everything is working fine but i can't for the life of me figure out how to access the rest of the space on my SD, when i insert the sd card into my PC it just shows up as the 115mb partition. i've tried using EASeus, the lexar bootit, and some other partition tool with no luck. with Easeus i select the 13gb FAT32 partition and make it active and thats it right? does it need to be logical? and which partition am i resizing?

None of the above. Windows will only recognize the first partition on a SD card, and when you make a SD bootable, that first partition will be the boot partition. To access the larger storage partition, connect your NC with CM7 running to the PC over USB. Open notifications on the NC, tap through "USB connected" to the USB screen, and "Turn on USB storage." Windows will now detect two storage drives, NookColor (the system partition, where apps install) and SDcard (the large storage partition).
The only time you'll want to remove the card from the NC and plug it into your PC is when you want to put on a new cm...zip update file to flash a new ROM.
I used the size agnostic install method with a 16gb PNY calss 4 SD and after all the steps were completed I was showing 13.8gb free for my SD card when I looked at in on the NC. Not believing my eyes I pulled it out and looked at it with EASUS and it showed partition 4 as 13.8gb (utilizing all the cards remaining space). I then downloaded a few things and added a few books for my wife and the space shrank to 13.4gb. As stated above your computer wont show it.
In short have you looked on the NC under "Storage" (I think that is were I found it) and verified you don't have the full capacity already?
Also there is another method I needed to use when I was messing with an earlier version (Nookie) that allowed me to see an SD card as a "Local Device" so windows would let me see all partitions on the hard drive. I can't post a link until I have more posts but email me and I can send you the link if interested.

thanks, well that got me some where. i connected nook to PC, turn on storage mode and it shows E/F drive. click either one and it says "Please insert disk into removable disk E/F" so then i turn off debugging and the nook color internal memory pops up as drive F, while clicking on drive E gives me the same error as above. any ideas? btw i'm on Windows 7 64bit, do i need any kind of special drivers or anything?
It is because windows will only allow you to manipulate the first partition on an SD card...even if it "sees" the other partition as another card it will not do anything with it. EASEUS will show you all the partitions on the SD.
The only way I got Windows to see all the partitions and do anything with them was to change my driver via the "Hitachi fix" If you google Hitachi driver SD card you will find it. I used this site and even though it is long it did exactly as advertised and now I install this driver when I need to format just the partition of an SD card and then "roll back" the driver when I'm done. It tricks windows into thinking it is a local device instead of removable. www/1src/com\forums\showthread.php?t=133718

I can't post a link as I'm still a noob...

S/he really doesn't need Windows to do anything with those other partitions--I suspect messing with them was where s/he went wrong in the first place.

I suggest you start over, because you probably damaged your install when you were messing with the partitions earlier. I'm also using Win7x64, and no, I didn't need any special drivers. Delete all partitions except "SDcard" in EASEUS, expand that partition to the whole card, then write verygreen's image to the card again and re-install. At that point, you should be able to access storage over USB from CM7 without turning off debugging or taking any other special steps other than "Turn on USB storage."

alright i'll give it a shot when i get home from work. is the verygreen image the same one found in the CM7 size agnostic SD thread?
and when i delete all partitions except for "SDCARD" do i need to make that partition logical or active or anything?
again thanks for your help.

No, in fact you might be able to write the image without messing around in EASEUS at all--I'm just not sure if WinImage (or whatever) would write to the whole card or just one partition. I know if you tried to format it, Windows would only format the boot partition.

16gb Micro Sd wont delete or add Items but I can open files but cant format the sd?

So I have this Issue I found my 16gb HD2(I no longer have the hd2) that's I guess corrupted I can view the files move them to my desktop but I cant format the SD, I tried my Nexus One, digital camera, Windows 7, Macbook, ubuntu, mini partition tool and testdisk with no results. I remember this issue happen when I tried to run android through SD maybe you guys know why. thanks
Some sd cards especially ones that have been bought on ebay or overseas have a memory hack in them. What happens is the sd card shows 16gb but in reality it may only have 6-10gb. The hack provides the sd card to show more space then is actually there. You can use it just fine with no problems at all but once you go over the actual memory it really has & not just shows you then get a slew of corrupted files on your sd card leaving it almost useless. This is only 1 example, others causes could be if you take an ad card out while you transferring files to it. In the end there is little you can do and what I would of suggested you've already tried. (minitool) this has helped me fix a card that I was transferring files when my power went out. This will probably not help but have you tried different ports on your computer? What happened when it was in you nexus?

Yeah I learn this lesson on my chinese 32gb lol but on my 16gb sd was actually the one that came with the hd2 so I know for a fact is legit. everytime I put it on my N1 it tries to format it and then a message pops out saying card unmounted. I also tried my good old nokia n82 which usually Nokias are really good at formatting corrupted cards it doesn't give me the choice of formatting the card. Im wondering of their some type of lock on the sd card.
there is a really small lock on the front of a sd card but none that i know of on a micro sd. did you use the minitool partition wizard or the data recovery program? if you only used the partitioning wizard i would try the minitool data recovery program found here.
Running mini tool this is what I get it wont fix it though https://picasaweb.google.com/lh/photo/3-oc0r_Bbv5qrYncR3IzCZaCRbbB6vENK-AyJ2O_CkI?feat=directlink
running minitool data or minitool partitioner? it looks like minitool data if i remember correctly. yeah, it says "minitool found errors on disk but will not fix them because diskcheck was run without fix parameters." i would try it again but select "check and fix detected errors" see if that helps. if it dont help then i would suggest to wait for advise from someone a little more knowledgeable about this.

what about good ol' Check Disk from Windows Explorer?
Another program is SD Formatter 3.0.
Try it and play with the options.
SD formatter is pretty good i must say. Here is another one that might be of some use. I have a corrupted flash drive around somewhere so i'll try and find that. maybe I can play around with it to see what might work and report back. I would also take a look at this link as it provides some very useful information about sd cards. you can find that here, please make sure you read the section on "Formatting is required". This will explain why you may be running into problems along with what the cause might be.
Yea the same thing happened to me with my 8 gig card, i even tried formatting it at best buy and with a camera and still wouldn't format. So i just had to buy a new one.
